Club registration form: template and online signing

Every season, the club has to collect registration forms from dozens or hundreds of players. If families fill them in and sign them on their phone, you save on paper, illegible handwriting and forms that never come back.

In 30 seconds

  • Only ask for the details the club needs: player, family, contact, relevant health information and consents.
  • If the player is 14 or over, they can consent to the use of their data themselves (under Spanish law); if not, their parents sign.
  • Put image consent in separate tick boxes so the family can choose.

What details to ask for on the form

  • Player: name, date of birth, ID number if they have one, age group and team.
  • Father, mother or guardian: name, ID number, phone and email.
  • Relevant medical information (allergies, medication), only what's needed.
  • Image consent (separately, see the image rights consent template).
  • Acceptance of the club rules and the data protection notice.

How to send it to every family

  1. Save the form as a PDF and upload it at Sign for free.
  2. Tap "Name", "ID number", "Phone", "Tick box" and "Signature" and put them in place.
  3. Send it to each family by email or WhatsApp. If someone forgets, we send them reminders.

FAQs

Do families have to sign up?

No. They get a link, open it on their phone, fill in the details and sign.

Do both parents sign?

If you need them to, yes: add two people to the same document and each one gets their own link.
